Financial Navigation Program

The Chronic Care Collaborative Financial Navigation program is designed to address the effects suffered by patients who have difficulty coping with the cost of their medical care – not only financial distress and occasionally financial ruin, but adverse health impacts as well. Inability to pay for needed care is a principal contributor to patients declining that care, or dropping out of care altogether.

With the assistance of a grant from the Adira Foundation, we are pleased to offer financial navigation services at no cost to patients diagnosed with neurodegenerative diseases, including Parkinson’s disease, Alzheimer’s, ALS, Multiple Sclerosis (MS), and Huntington’s disease.

Here’s How It Works

Step 1 (Outreach): Referrals will be received from providers as well as each of the member organizations of the Chronic Care Collaborative representing the neurodegenerative disease community. Our program is open to all patients of regardless of income or insurance status, as many patients do not recognize they are underinsured until they are facing expensive medical treatment.

Step 2 (Referral): When a referral is received, one of our expert financial navigators will reach out to the patient to learn more about what the patient is facing both medically and financially.

Step 3 (Navigation): Our navigators will then work to create an individualized plan for each patient first and foremost to ensure that any financial barriers to treatment are removed, and
then to reduce the financial burden on the patient so they can focus on getting well. The strategy might include insurance optimization, copay or premium assistance through foundations or pharmaceutical companies, free or reduced cost drug, enrollment in government programs, bill negotiation and resolution and community grants, among others.
