Who We Are & What We Do

The Chronic Care Collaborative (CCC) first began meeting in 2009 after realizing that most voluntary health organizations were advocating for similar healthcare goals. The CCC started with 24 organizations and 7 priorities, with the goal to represent and jointly advocate for the over 60% of Coloradans living with a chronic disease. For the past 15 years the CCC has successfully advocated for policies, regulations, and legislation which directly impact the lives of Coloradans living with a chronic disease. From advocating for expanded access to affordable healthcare services to offering respite care for caregivers, the CCC is dedicated to increasing life enhancing opportunities for Coloradans living with chronic disease, and their caregivers.
