Welcome to the Chronic Care Collaborative blog! The CCC is made up of 37 voluntary health organizations that serve as Colorado’s advocates for those living with chronic disease and their caregivers. This blog is dedicated to the 1 in 4 Coloradans living with chronic disease and is a needed voice in healthcare which will advocate for access to affordable coverage and care regardless of health condition.

The CCC has five principles which guide our advocacy work and are the foundation of the Collaborative blog. You will see frequent posts from different Collaborative members around integrated home and community based services, behavioral health services, Medicare and Medicaid issues, access to affordable coverage and care, and promotion of prevention and wellness.
